TOHS requires a solid casket enclosure, manufactured with ABS Plastic, that is
hermetically sealed and has a relief valve. If a container meets these specifications,
it will be permitted in our mausoleum. “Kryprotek” is an approved Liner.

Additional Requirements
1- Notification must be to 609-296-2886, leave a message, your call will be returned to confirm or send an email to greenwood08087@gmail.com and chris@mathisconst.com and confirm.
2- Minimum of 72 Hrs. notice required prior to interment with Deed, up to 5 days without Deed.
3- Copies of Interment Form, Burial Certificate, Cremation Certificate, Affidavit of Authority, Death Certificate, and Deed or Purchase Agreement are required to be provided by mail or email at least 72 hours prior to interment/entombment or disinterment.
4- Temp Metal Marker Required at time of Interment.
5- See Rules & Regulations for limits on Interments/In-Ground or Cremains.
6- All other Rules & Regulations of the TOHS must be observed.
7- All fees must be paid in full prior to interment.
8- Current Insurance Certificate naming TOHS as additional insured is required.
9- Minimum of 48 hours notice of any work in the cemetery, setting monuments, or engraving.
10- All Engravings are warranted by the Contractor completing the work, not the TOHS.
11- All Engraving Work requires a permit, work on existing markers permit fee $50.00
12- Must complete “Monument Work Permit” including Affidavit of Authority on our form
and receive approval prior to placement of monuments or stones.
13- Layout of Plot/Lot Corners $100.00
14- Multiple Interments of Cremains at the same time (Crypt/Niche/Burial) will require multiple fees.
Requires payment of TOHS fee, M & P fee & State fee times the number of internments at the same time, plus the number of opening fees that are required.
Crypt/Niche interments where engraving is included will require an additional engraving fee.
15- Affidavit of Authority & Deed is Required for any and all work in Greenwood Cemetery
16- Crypt or Niche, opening for any reason even if only to add cremains is full opening costs as listed on the price list.
17- ALL containers utilized to inter ashes in the Mausoleum must be engraved with the full name, DOB, and DOD of the person being interred. In lieu of engraving a Permanent weather-resistant to moisture, sunlight, etc. label may be used on interior and exterior, subject to the approval of TOHS.
18- Cremations: Additional cremations may be added at the time of another opening.
